Before you begin this assignment:

Please complete any required process at the facility where you will do your project for the facility/organization IRB or upload a letter stating an IRB review will not be required. Please submit any documents you have received as part of the facility IRB review process before you begin the submission of the USA IRB application. If you have not completed the facility/organization IRB process, please work with your faculty to determine when it is recommended that you submit the USA IRB application, In most cases, you will be asked to complete the facility/organizational IRB process, if a review by the facility/organization IRB is required, before submitting to the USA IRB.

For this assignment, students will submit the email confirmation of the successful IRB application submission as a PDF to the assignment submission link AND evidence the facility/organization IRB process has been completed or is not required.

Assignment on IRB Application

All students must complete an IRB application and receive approval BEFORE implementing the DNP Project. Log in to IRBnet.org  (http ://irbnet.org/) and complete the DNP Application (Part A, Part B, and all required attachments). Students may complete Part A prior to Part B (actual project application). The number of required attachments may vary depending on the student’s DNP project focus. All students are required to submit the following attachments:

1. NIH certificate (unless previously submitted)
2. DNP Application Part A
3. DNP Application Part B
4. Letter of Support from the agency/organization where the project will be implemented
5. Process Map
6. Participant Information Sheet (using the IRB template found in IRBnet.org)
7. HIPAA certificate (unless previously submitted)

The Faculty Advisor MUST review and electronically sign the student’s IRB Application prior to submission to IRBnet. Right click links to document examples below and open in a new tab.

Grading Criteria

NIH certificate (unless previously submitted) and DNP Application Part A are correctly submitted as part of the IRB package.

DNP Application Part B is correctly submitted as part of the IRB package.

Letter of Support from the agency/organization where the project will be implemented and Process Map are correctly submitted in the IRB package

Participant Information Sheet (using the IRB template found in IRBnet.org) is correctly prepared and submitted with the IRB package.

HIPAA certificate (unless previously submitted) is available with the IRB package.

The IRB package reflects graduate level scholarly work consistent with the directions provides from the IRB for CON DNP submissions.
