The purpose of this assignment is to create a draft logic model connecting your needs assessment and draft program outline.

For this assignment, you will create a logic model for your program and explore potential methods of evaluation. You will connect the evaluation method to the short and long-term goals for your program.

Instructions:

  1.  Provide a brief description of your program, the overall need or gap being addressed, and what you are hoping to evaluate. Include questions you have, what you hope to measure/evaluate.
  2.  Explore two potential evaluation sources – these can be quantitative, qualitative, or a combination. Include a brief description of each and how you plan to use them (e.g., surveying participants, measuring goals, how often they evaluations will be administered, when throughout your program (before, during, after), etc.) along with APA citations. Also include sample questions, surveys, etc. as an appendix at the end of the document.
