Windshield Survey Community Class

For the Windshield Survey Assignment, students will drive around the community of interest recording observations. The survey is typically conducted from a car, but could be carried out via any mode of transportation or via walking.  Set aside at least 2 hours to get a full picture of the community and avoid conducting the survey after dark.   The aim of this assignment is to get a complete picture of the community based on subjective and objective data, then create a care plan/community diagnosis complete with interventions and expected/projected outcomes. Introduction – What is known about the community?  Include Geographic location, zip code and map.  Provide a broad description of community of interest.

Subjective–  Description of people (gender/ethnicity/clothing/general appearance), landscape, schools, animals, public/private structures, common areas, transportation, stores, services, housing, community growth, politics/media, and possible threats to health (environment or otherwise).

Objective – Provide a detailed description of health care statistics for the region.  Include incidence and prevalence of diseases, cultural and religious demographics, education background, known threats to health

Nursing Diagnosis/Care Plan – Create a community care plan based on identified needs.  Focus on the two highest priorities and develop at least two interventions for each.  Provide a brief section about Evaluation – how will the interventions be measured and at what intervals?  What does a successful intervention look like for the community?

Literature – Integrate literature into each section.  For example, how do selected interventions dovetail with Healthy People 2020, WHO, Monroe County Health Department, etc.?
